Any NYCERS member who participated in World Trade Center Rescue, Recovery, or Clean-up must fill out this (F622) form, have it notarized and submit it to NYCERS before 9/11/2026. This form must be on file (even if you are on 3/4 WTC disability). Without this form on file, if you pass from 9/11 illnesses, you cannot get your NYCERS pension reclassified as Line of Duty and your spouse will not get free medical for life. If the member has already passed, their widow may fill out the form on their behalf. Submit this form as soon as possible and share with any other NYCERS members you may know.
